Why the steps are four weeks

Semaglutide has a half-life of about a week, which is why it is dosed weekly and why each escalation step is held for four weeks. Four weeks is roughly four half-lives, the point at which levels from a new dose have substantially settled. Stepping faster means changing the dose before the previous one has finished arriving.

The 0.25 mg start is not a treatment dose

It exists to let the gastrointestinal effects settle. This is the most misread part of the schedule: people conclude the compound is not working during a phase that was never intended to do anything except be tolerated.
